Form 1096 is the paper cover sheet that carries information returns — Forms 1097, 1098, 1099, 3921, 3922, 5498 and W-2G — to the IRS (Form 1096 instructions, Purpose of form). It summarizes and transmits the underlying information returns; it does not replace them. Electronic filing does not use Form 1096. Do not send forms that contain summary or subtotal information with Form 1096; Boxes 3, 4 and 5 summarize the group of underlying returns being transmitted (Transmitting to the IRS).

When Form 1096 applies to your firm

Use Form 1096 when your firm mails paper information returns. Before assembling anything, confirm the delivery channel for each return type and the aggregate volume for each filer, because the paper route depends on each filer's total efiling obligation for the whole year rather than on one form type. Determine the aggregate efiling obligation for each filer, including applicable exceptions, because filing 10 or more information returns during the year may require electronic filing instead of paper (Box 4 caution).

One transmittal per form type

Each form type being filed requires its own Form 1096 (Transmitting to the IRS). A stack of Copy A/IRS copies of 1099-NEC, 1099-MISC and 1099-INT goes into three separate transmittals, not one. W-2 wage reporting travels on its own path: Form 1096 covers Form W-2G, which reports certain gambling winnings, and is unrelated to Form W-2 wages.

Originals and corrections of the same return type can travel together on one Form 1096 (Corrected returns). That flexibility is useful in January and February, when a corrected 1099-NEC is processed alongside the original batch.

Return type Transmittal How Box 3 is completed
Form 1099-NEC Form 1096 Count of correctly completed 1099-NEC forms, excluding blanks and voided forms
Form 1099-MISC, 1099-INT, 1099-DIV Form 1096 Count for that form type only, since each type needs its own transmittal
Form 1098 Form 1096 Count of 1098 forms
Form 5498 Form 1096 Count of 5498 forms
Form 3921 / 3922 Form 1096 Count of the applicable form type
Form W-2G Form 1096 Count of W-2G forms
Form 1098-T, 1099-A, 1099-G Form 1096 Box 5 requires no entry (Box 5 instructions)
Form W-2 wages Not on Form 1096 Wage reporting follows its own filing path
Electronic filing No transmittal Form 1096 is not used (Transmitting to the IRS)

Box 3 asks for the number of correctly completed forms you are transmitting with that transmittal. Blank forms, voided forms and the Form 1096 itself are excluded from that count (Box 3 instructions).

Filling the cover sheet

Work through the identifying block first: filer name, address and taxpayer identification number as they should appear on the underlying returns. The remaining operational boxes follow in sequence — total forms, Box 4 for the federal income tax withheld, and Box 5. The caution about a possible efiling requirement is a note on the form, not a box function. Box 5 asks for the total dollar amount from the specific boxes set out in the instructions for that return type; for Form 1098-T, 1099-A and 1099-G, no entry is required (Box 5 instructions). Check that rule against the specific return type on your transmittal rather than applying it across the whole package.

Assemble the package so the transmittal sits with the returns it describes, and the Copy A/IRS copies of the underlying returns remain in their correct order with the count in Box 3 matching the physical contents.

The scannability trap

The downloaded form itself is the trap. A Form 1096 downloaded from the IRS website and printed from the PDF is not scannable, and filing it may result in a penalty (Attention filers notice). Instead, use IRS-provided scannable stock or an approved substitute that meets the applicable IRS specifications.

Due dates on the transmittal

The transmittal does not carry a single universal deadline. Due dates vary by the underlying return (When to file). Form 1099-NEC must be filed with Form 1096 by January 31 (When to file); the surrounding paper workflow is covered in Form 1099-NEC and related information returns. Forms 5498 must be filed with Form 1096 by May 31 (When to file). The current form lists Form 1099-LPS separately from the general February 28 schedule, so build your calendar from the form in hand rather than from a memorized date (When to file). A request to extend time to file is a separate filing handled on Form 8809, described in our Form 8809 guide.

When a due date falls on a weekend, Sunday or a legal holiday in the District of Columbia or in the filing location, the deadline rolls to the next business day (When to file).

Frequently asked questions

Does Form 1096 apply to electronic filing? No. Electronic submission does not use a Form 1096 transmittal (Transmitting to the IRS). The form exists to accompany paper returns.

Can I combine corrected returns with originals? Yes, when they are the same return type. Originals and corrections of the same return type can be submitted using one Form 1096 (Corrected returns). Different form types still require separate transmittals.

Are all 1099 returns due February 28? The general February 28 schedule is not universal. Form 1099-NEC must be filed with Form 1096 by January 31, and the current form lists 1099-LPS separately from the general February 28 schedule (When to file). Form 5498 has its own May 31 date.

What happens if I print the downloaded form? A Form 1096 printed from the IRS website PDF is not scannable, and filing it may result in a penalty (Attention filers notice).

How is Box 3 determined? It is the number of correctly completed forms you are transmitting on that transmittal, excluding blank forms, voided forms and the Form 1096 itself (Box 3 instructions).

Where should our firm start? Review your next paper batch and check three things: whether each return type has its own transmittal, whether the print source is scannable, and whether the deadline was derived from the underlying return and your calendar. Firms that want a systematic review can work through the guides library alongside a walkthrough of their process.
