If your tax refund was issued but never arrived, Form 3911 helps you document the problem and request an official trace. This form does not speed up processing of a return still under review, nor does it guarantee you will recover missing funds. It provides the IRS with the information needed to investigate what happened to a refund that was already released.

What Form 3911 Does

Form 3911, Taxpayer Statement Regarding Refund, is the official document for reporting a refund you expected but did not receive—whether the check was lost in the mail, stolen, destroyed, or a direct deposit failed to appear in your account. According to the IRS guidance at official IRS guidance, the form supplies facts necessary to trace an already issued refund. It does not unlock a return held for examination or remove a processing freeze. If your refund is simply delayed because the IRS has not finished processing your return, Form 3911 will not accelerate that timeline.

The trace process investigates the payment trail: when Treasury issued the refund, where it was sent, whether a check was cashed or a deposit rejected. Depending on what the trace uncovers, the IRS may reissue the refund or inform you that the original payment was successfully delivered and no replacement is warranted.

Completing and Submitting the Form

Prepare a separate Form 3911 for each missing refund. You will need to provide your name, Social Security number or taxpayer identification number, current address, the tax year and form number of the return that generated the refund, the refund amount, and details about how the refund was supposed to arrive, including available check and mailing details or applicable bank information requested by the form.

Mark the section that describes your situation: refund not received, check lost or stolen, check destroyed, or deposit issue. Both spouses must sign Form 3911 for a joint-return refund. Automated systems cannot initiate a joint-return trace; use the IRS phone route or complete the form as directed.

Do not rely on an address printed on an old copy of the form. The IRS updates mailing addresses and fax numbers based on your state and the type of return. Consult the current routing table at official IRS guidance to find the correct destination for your submission. The form itself is available at Form 3911 (PDF). The listed fax numbers accept only Form 3911; do not send other tax documents to those fax lines. Keep a copy of your completed form and proof of mailing or fax transmission for your records.

Direct Deposit Problems

If you expected a direct deposit that never arrived, contact your bank or credit union first. The IRS provides situation-specific guidance at official IRS guidance and official IRS guidance for incorrect or missing deposits. Verify the account details you provided on your return, confirm whether any deposit was received and returned, and ask the financial institution to research the transaction.

For the direct-deposit situation described in the IRS FAQ, if you have contacted the financial institution and five calendar days have passed with no deposit, follow the Form 3911 trace instructions. Banks are allowed up to 90 days from the initial trace input to respond, and the entire resolution process can extend to 120 days depending on the complexity of the investigation. These are not universal deadlines for all refund situations—they reflect the time frames described for certain direct deposit traces. The IRS cannot force a financial institution to return funds that are no longer available or that the institution refuses to release, so recovery is not guaranteed.

Decision Points for Refund Traces

Situation Check first Appropriate next step
Issued paper refund not received Issue date, payment method and current IRS trace guidance Use an eligible self-service, phone or Form 3911 route
Check lost, stolen or destroyed The actual refund and what happened to the check Follow IRS instructions for a trace; replacement is not guaranteed
Direct deposit missing or misdirected Bank records and the return’s payment details Contact the bank and follow situation-specific IRS instructions
Joint-return refund missing Both spouses’ information and required signatures Contact IRS or complete Form 3911; automated initiation is unavailable
Return still under review The current processing or notice status Follow the review instructions; a trace does not release the review

Related Situations

If your return is under review rather than simply missing a refund, you may be dealing with a different kind of delay. For amended returns, see IRS Amended Return Status: What to Check Next to understand processing timelines. Identity verification requests are covered in IRS Letter 4883C: Prepare for Identity Verification, and income, withholding and credit review is explained in IRS CP05 Notice: Review Timing and Next Steps. Form 3911 is not a substitute for responding to those notices.

How long does a refund trace take?

Time frames vary depending on whether the trace involves a paper check or a direct deposit dispute. For certain direct deposit situations, the bank may have up to 90 days to respond to the IRS inquiry, and resolution can take up to 120 days. These are not universal deadlines and do not apply to every refund trace.

Can I file Form 3911 if my refund is still processing?

No. Form 3911 is for tracing a refund that was already issued. If your return is still under review or has not been processed, the form will not accelerate that process or release any holds on your account.

What if the trace shows the refund was delivered correctly?

The IRS may determine that the check was cashed or the deposit was accepted by the account you provided. A cashed check or posted deposit does not by itself resolve every dispute, including possible forgery. Follow the trace findings and the IRS or financial institution’s instructions; replacement or recovery is not guaranteed.
